IV/IM Adult 2-4 g in equally divided doses every 12 hr. Severe or refractory infections May be increased up to 8 g in equally divided doses every 12 hr. Max dose: Sulbactam 4 g daily. Childn 40-80 mg/kg daily in equally divided doses every 6-12 hr. Serious or refractory infections May be increased up to 160 mg/kg daily in 2-4 equally divided doses. Neonateup to 1 wk Max dose: Not to exceed sulbactam 80 mg/kg daily every 12 hr. CrCl 15-30 mL/min Max: Sulbactam 1 g every 12 hr, <15 mL/min Max: Sulbactam 500 mg every 12 hr.
Discontinue use if allergic reaction occurs. Risk of vit K deficiency eg, patients w/ poor diet, malabsorption states (cystic fibrosis) & on prolonged IV alimentation regimens. Overgrowth of nonsusceptible organism due to prolonged use. Monitor prothrombin time in patients receiving anticoagulant therapy & exogenous vit K. Periodically check organ system dysfunction in extended therapy. Severe biliary obstruction &/or hepatic disease or coexistent renal dysfunction. Pregnancy & lactation. Premature infants & neonates.
